FailSafe for Life: Recognition and Referral Suicide Prevention Training

Presented by Sabrina Votava, Executive Director, FailSafe for Life

Length: 3-hour interactive training

Dental professionals often build long-term, trusting relationships with patients and may be among the first to notice signs of emotional distress. FailSafe for Life prepares dental teams to recognize suicide risk during patient interactions and respond in a supportive, professional manner.

This training addresses the scope of suicide, warning signs, risk and protective factors, and symptoms of depression that may present in dental settings. Participants will learn to use the PHQ-9 screening tool and follow practical intervention steps—engage, ask, stay, remove, and connect—with an emphasis on means safety and appropriate referral. Case examples and guided practice focus on real-world dental scenarios, helping participants gain confidence in having compassionate conversations and connecting patients to local and national resources. Full attendance is required for a certificate of completion.

Sabrina Votava is an educator on suicide prevention and intervention who has worked with hundreds of individuals and organizations around the country to help them recognize the signs of suicide and to give them the tools and resources to keep a person in a mental health emergency safe.  Having worked in this field for over two decades, Sabrina’s passion is evident in her trainings.


Her personal experiences coupled with years of research give her the ability to connect with a wide variety of audiences from youth to adults, nonexperts to professionals.  Sabrina has been featured in various media outlets throughout Washington state over the years and is the founder and Executive Director of a Spokane-based suicide prevention non-profit named FailSafe for Life.  She is a licensed mental health counselor, certified as a QPR Master Trainer and ASIST trainer, and is the creator of FailSafe for Life’s various trainings. She is also a member of the Leadership Team of the Prevent Suicide Spokane Coalition.

Sabrina holds a B.S. in Public Health from the University of Washington and an M.A. in Social and Behavioral Health from Whitworth University.
